Wisdom tooth pain: causes, relief and when to see a dentist

Wisdom teeth are the last to arrive and often the first to cause trouble. Here is why they hurt, how to ease the pain tonight, and how to tell whether they need to come out.

The last teeth, and the most troublesome

Wisdom teeth, the third molars at the very back of the mouth, usually come through between the late teens and mid-twenties. In many people the jaw doesn’t have enough room for them, so they come through at an angle, only partly break through the gum, or stay trapped in the bone. Wisdom tooth pain is one of the most common reasons young adults visit a dentist in a hurry.

This guide explains the six main causes of wisdom tooth pain, what you can do at home tonight, the warning signs that need urgent care, and how a dentist decides whether a wisdom tooth should be kept or removed.

6 common causes of wisdom tooth pain

1. Pericoronitis

Food and bacteria trapped under the gum flap over a part-erupted tooth cause infection, swelling and pain.

2. Teething pressure

A tooth pushing through the gum can ache and make the gum tender, even without infection.

3. Decay

Wisdom teeth are hard to clean, so they and the tooth in front often decay.

4. Impaction

A tooth trapped against the next tooth can press on it and cause a dull, deep ache.

5. Gum disease

Pockets around back teeth trap bacteria and become inflamed.

6. Cysts

Rarely, a fluid-filled sac forms around an impacted tooth and damages the bone.

What wisdom tooth pain feels like

Wisdom tooth pain can range from a mild, on-off ache at the back of the jaw to sharp, throbbing pain that spreads to the ear, throat or temple. You may notice a swollen, red gum behind your last molar, a bad taste, bad breath, difficulty opening your mouth fully, or pain when you swallow. Biting on the swollen gum flap makes it worse, which is why the pain often flares up after meals.

Some people also notice swollen glands under the jaw, a mild temperature or a general feeling of being unwell when the gum is infected. Pain that comes and goes for several months is a sign the tooth is repeatedly causing trouble, even if each episode settles.

Quick relief at home

These steps can ease pain from a wisdom tooth until you can see a dentist. Rinse your mouth with warm salt water several times a day, especially after eating, to wash food out from under the gum flap. Brush the area gently with a soft brush, even if it’s tender. Take an ordinary painkiller exactly as directed on the pack, and check with a pharmacist if you take other medicines. Hold a cold compress against your cheek for fifteen minutes at a time.

Avoid chewing on that side, and choose soft food for a few days. These measures calm the inflammation, but they don’t remove the cause, so book a check-up even if the pain settles.

Please do not

  • Put aspirin or tablets on the gum
  • Apply heat to a swollen face
  • Use leftover antibiotics
  • Ignore swelling that’s spreading
  • Take more painkiller than the pack allows

When wisdom tooth pain is urgent

Call us the same day if the pain is severe, if your gum or face is swollen, if you have a bad taste or pus, or if it is hard to open your mouth. These are signs of infection that usually needs cleaning, sometimes medication, and a plan for the tooth.

Go to a hospital emergency department, or call 998, if swelling spreads towards your eye or down your neck, if you have difficulty swallowing or breathing, or if you have a high fever with facial swelling. Infections from lower wisdom teeth can occasionally spread quickly and need urgent medical care. See our emergency dentist page.

How the dentist finds the cause

The dentist looks at the gum around the tooth, checks the tooth in front for decay and takes an X-ray. The X-ray shows the angle of the wisdom tooth, how much room there is, whether it’s decayed and how close the roots are to the nerve in the lower jaw. This tells us whether the tooth is likely to come through properly or keep causing pain from a wisdom tooth.

Should the wisdom tooth come out?

Not every wisdom tooth needs removing. If it has come through straight, is easy to clean and isn’t causing problems, it can stay. Removal is usually recommended when a tooth causes repeated infections, decays and cannot be repaired, damages the tooth in front, or is linked to a cyst or gum disease.

If the tooth is only causing occasional mild pain from a wisdom tooth, we may suggest cleaning, careful monitoring and a review. We always explain the reasons, the risks and the alternatives so you can decide.

Removal is often advised if

  • Infections keep coming back
  • The tooth is decayed and can’t be fixed
  • It damages the tooth in front
  • It causes gum disease or a cyst
  • It’s impossible to clean properly

What wisdom tooth removal involves

Most wisdom teeth are removed under local anaesthetic in the dental chair. Teeth that have fully come through can often be removed much like any other tooth. Impacted teeth may need a small cut in the gum and the tooth may be removed in sections. You’ll feel pressure but not pain. Very difficult cases, or those very close to the nerve, may be referred to an oral surgeon. Read more on our tooth extraction page.

Recovery in brief

Swelling and stiffness usually peak on the second or third day and settle within a week. Bite on gauze for the first hour, avoid rinsing, smoking and straws for the first day, eat soft food, and start gentle warm salt water rinses from the next day. Contact us if pain gets worse after day three, which can be a sign of dry socket. For more detail, the NHS guide to wisdom tooth removal is a helpful independent source.

Can wisdom tooth pain cause headaches or ear pain?

Yes. The nerves in the back of the jaw are close to those supplying the ear, temple and throat, so pain from a wisdom tooth is often felt as earache, a headache on one side, a sore throat or a stiff jaw. People sometimes visit a doctor for ear pain that turns out to be dental. If you have ear or jaw pain with a swollen gum behind your last molar, see a dentist.

Preventing problems with wisdom teeth

You cannot stop wisdom teeth from growing, but you can reduce the chance of infection. Brush right to the back of the mouth twice a day, angling a small-headed brush behind the last molar. Rinse with warm salt water after meals if the gum is partly covering the tooth. Keep up check-ups, because X-rays show developing problems long before they hurt, and decay in the tooth in front can be caught early.

If you’re planning to travel, start a new job or sit important exams, it can be worth having a troublesome wisdom tooth assessed beforehand rather than risking a painful flare-up at a bad time.
